Conditions of Enrollment in Wellness Program

Equine Veterinary Service’s Wellness Program

  • Payment in full is required at the time of enrollment.  In order to provide cost savings to our clients, account balances must be paid in full before enrollment in the program.  Visa, Mastercard, Discover, American Express, Checks and Cash are accepted.
  • The enrollment period will remain open until January 31st, 2022, or until the program is filled, so please enroll early. Enrollment for your horse(s) is secured upon full payment of the Wellness Program. A quote does not guarantee enrollment.
  • Clients are responsible for calling the office to schedule appointments.  We strongly recommend scheduling your Wellness Program appointments at the time of enrollment for optimal choice and availability.  We do our best to accommodate specific doctor requests, but cannot guarantee the availability of an individual veterinarian for an appointment.
  • Farm call fees are included in the price of the program, but due to variability, pricing will be determined at the time of enrollment/payment.  Farm call fees are determined by location and number of clients splitting the farm call at one location.
  • Any additional services requested/provided at the time of the Wellness Program visits are due and payable at the time of the visit.
  • Credit card authorization is available if preferred, or if a client cannot be present.  Note that for the provision of additional services, written or verbal authorization is required by the client if they cannot be present when services are rendered.
  • Boosters for initial vaccination series are not included in the program & are charged separately.  Boosters will require a separate visit and additional farm call (not discounted).
  • If you do not utilize all the services included in your Wellness Program, there is no refund or substitution made for your unused opportunities.
  • If your horse is sold horse before fulfillment of the Wellness Program, the remaining services of the program may be transferred with the horse and be available for use by the new owner if they are located within the E.V.S. practice area.  No refunds or substitutions will be made for unused opportunities.
  • In the event your horse should pass away before fulfillment of the Wellness Program, we will credit your account for the unused, pre-paid services.
  • If a client’s account does not remain in a “current” financial state, any remaining Wellness Program monies may be utilized to bring the account current.
  • To ensure adequate time to evaluate your horse, unscheduled horses will not be seen during a Wellness Program visit unless scheduling permits.
  • A separate appointment is required for dental work under the plan unless scheduling permits otherwise.
